Ahead of a full consultation, a call for evidence has been launched as part of a broader initiative to reform the fit note. Originally introduced in 2010 to replace the sick note, the fit note emphasised the benefits of returning to work and provided employers with essential information to support an earlier return to work. This reform enabled GPs to suggest that a worker “may be fit for work subject to the following advice.”
Current Challenges with Fit Notes
From October 2022 to September 2023, only 6.2% of fit notes utilised the option to offer advice for returning to work, resulting in over 10 million being issued without any guidance to facilitate re-entry into the workplace. The call for evidence highlights a critical issue: the current process does not adequately reflect the diverse needs of individuals regarding how health conditions impact their work capability. This shortfall is attributed to a lack of capacity and specific expertise in work and health within primary care, as well as the absence of a clear pathway to refer individuals to more specialised or intensive support when necessary.
Proposed Changes to Fit Note Issuance
The proposed reforms aim to streamline the fit note process by:
- Redirecting patients who require a fit note to a designated professional within the NHS, rather than their general practitioner (GP).
- Ensuring this professional is specially trained to discuss the impact of health on work capabilities.
- Providing advice on how to return to work with support from employers.
- Empowering this professional to issue a fit note if additional time off is necessary.
